	Organization Code: This should be easy to remember for everyone in your organization. For example, choose your organization's nickname, colors, icon or street name.
	Administrator Password: Any set of numbers or letters different from the organization code. This will allow you to personally open your administrator's page to view your organization's sales and to access Givenik update to distribute.


	To participate in Givenik, an organization must be 501c3 or 509a1 tax exempt, currently in operation and located within the United States. Check this box to confirm your organization complies.

APPLYING TO BE A GIVENIK MEMBER ORGANIZATION Applying to become a member organization of Givenik is a simple two step process. A representative of the applicant organization legally authorized to agree to the terms of the agreement needs to fill out the form above accurately in its entirety. If the organization's application is accepted, the representative will be notified by email and the applicant organization will then be mailed a copy of the Agreement for the Organization to sign. The Organization Agreement needs to be signed and returned to Givenik by mail. An applicant organization becomes a member organization of Givenik once it has received a counter-signed copy of the signed agreement. Givenik reserves the right to deny, without cause, any application submitted by an organization for membership. The applicant organization is responsible for maintaining the accuracy of all business and personal information in its account and for protecting the security of its account. Givenik is not responsible for mistakes in any information provided by the applicant. The applicant organization may not at any time represent itself to be a member of Givenik or use or redistribute any materials of the Givenik.com website until it receives a counter-signed copy of the Organization Agreement. All communication and agreements between Givenik and the applicant organization are strictly confidential and may not be shared with any other party without the express, written consent of Givenik. Givenik is operated by Givenik LLC.
